What is a Primary Care Network?
Primary Care Networks (PCNs) are groups of GP practices working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local area. Introduced in 2019 as part of the NHS Long Term Plan, PCNs typically cover 30,000–50,000 patients and are designed to offer more proactive, personalised and coordinated health and social care.
Community-centred care
PCNs bring together local GP practices to collaborate at a neighbourhood level, ensuring care is tailored to the specific needs of the communities they serve.
Expanded workforce
Through the Additional Roles Reimbursement Scheme (ARRS), PCNs employ a wider range of health professionals including pharmacists, physiotherapists, social prescribers and physician associates.
Proactive health services
Rather than waiting for patients to present with problems, PCNs focus on prevention, early intervention and improving outcomes for people with long-term conditions.
